Mentoring For Kids

1. Counseling Intake. One of our counselors will complete an intake.

2. Psychological Evaluation. Our psychiatric nurse practitioner will conduct an evaluation.

3. Submit PRP Referral. We will submit your referral to Optum MD for a decision (takes 2 weeks for approval/denial).

4. Assignment of a Life Coach/Mentor. Children automatically qualify and will be assigned a mentor. Adults with a qualifying diagnosis will be assigned a mentor.

5. Treatment Plan. The mentor will assist in developing a treatment plan to guide treatment and stay on track with goals. 

6. Attend PRP Sessions. Adults must attend PRP sessions at least six times per month. Children must attend PRP sessions at least three times per month.

7. Attend Therapy Sessions. All mentees attend therapy at least twice a month.

• On-going progress assessments
• Discharge planning & follow-up
• Relapse prevention

Psychiatric Rehabilitation Program (PRP) is a free mentorship program for children who struggle with their mental health. To qualify for the program, the individual must have Maryland Medicaid and have an impairment such as an emotional disturbance or behavioral issue that affects their home and/or school life. The program is based on the recovery and person-centered models of rehabilitation. Through the program, children are provided with the resources and support to reach a higher level of independence, success, and fulfillment in their lives. 

By participating in the program, participants are provided with:

• A life coach/mentor to help guide them
• Skills and support to embrace their mental wellness
• Tools to enhance the quality of their life
• Encouragement and guidance to pursue personally valued roles in their living, learning, work and social environments
  
  

The purpose of the program is to:

• Promote recovery and resiliency
• Empower the individual so that they may function successfully and live as independently as possible in their community
• All PRP participants will receive high-quality, person-centered services tailored to their individual needs. With the support of their mentor, participants will live successfully in their community equipped with the tools and resources to address living, working, mental, emotional and social functioning.

As a result of having an increased access to mental health care services and support, it is our hope that all participants will have a reduced use of more intensive interventions.

• Active Maryland Medicaid health insurance
• Referral from a licensed mental health professional
• The youth’s impairment(s) and functional behavior can reasonably be expected to be improved or maintained by using these services.
• The youth’s emotional disturbance is the cause of serious dysfunction in multiple life domains (home, school, community)
• The impairment as a result of the youth’s emotional disturbance results in:
• A clear, current threat to the youth’s ability to be maintained in his/her customary setting, or
  An emerging/impending risk to the safety of the youth and others, or
  Other evidence of significant psychological or social impairments such as inappropriate social behavior causing serious problems with peer relationships and/or family members.
• The youth, due to the dysfunction, is at-risk for requiring a higher level of care, or is returning from a higher level of care.
• The youth’s condition requires an integrated program of rehabilitation services to return to age appropriate development and to progress accordingly towards independent functioning and independent living skills,
• The youth does not require a more intensive level of care and is deemed to be able to be safely maintained in the rehabilitation program and to benefit from the rehabilitation provided.
